I know this sounds like stupid question but what actually happens if people don't tap to a guillotine?

Just wondering if it would be a useful restraint tech in a street situation. Probably not right?

A properly applied guillotine, like most chokes, cuts off the blood flowing to the brain in the large arteries on either side of the neck. It causes unconsciousness within a few seconds if you don't tap. You'd have to hold it upwards of a minute post unconsciousness for there to be permanent damage.

A sloppily applied guillotine will be crushing the trachea with the forearm or cranking the neck. Either could do some permanent damage.
