That is a common kicking combination found in many styles.

I could be wrong, but the kicking and jumping kicks he does looks like it could be from the Chin Woo Jie Quan form. (Jeet Kune form). Lee learned the Jie Quan form from Shiu Hon Sang. Lee performed a part of that form with those stylish kicks many times including the 1964 Long Beach tournaments.

If im wrong, somebody correct me!

i heard from many masters that this from northern praying mantis but i dont know which form from this style?

I don't think that kick combo is in Jie Quan (http://www.youtube.com/watch?v=51Vw4d4pw-E&feature=related), though it is in the Chin Woo curriculum, somewhere.

So, if we are talking about Chin Woo and Northern Praying Mantis, then I would say Seven Star Praying Mantis, because Luo Guang Yu taught at the Chin Woo Shang Hai Central branch. His lineage of Mantis picked up some of the Chin Woo core forms and Mantis-ized them.

I can't say that Bruce lee knew any mantis though.

That kicking combo looks like 三不落地(San Bu Luo Di) or 五鳳齊飛(Wu Feng Qi Fei) in the 华拳(Hua Quan) 3rd Road. The reason it's called 三不落地(San Bu Luo Di) because you jump 3 times. In all 3 jumps one of your feet is always in the air.

- jump front kick,
- jump back heel kick,
- jump inside (or outside) crescent kick.

It's also called 五鳳齊飛(Wu Feng Qi Fei) because you make 5 slap sounds in this combo (Bruce Lee didn't do the slap sound).

- right fist hit left hand,
- right hand hit the instep of right foot,
- right hand hit the bottom of the left foot,
- right hand hit left hand,
- right (or left) hand hit the outside (or inside) edge of the right foot,

Bruce Lee ended this kicking combo with inside crescent kick. The outside crescent kick is more commonly used. Not sure about the combat value for this kicking combo but if you can still do this combo when you are 80 years old, you body should still be in good shape. IMO, it's a good "exercise for health" for your daily training. I strongly recommand.

I like to add left side kick, right backward floor sweep, left foot sweep, and left elbow strike at the end of this triple jumping kick and end with a nice physical challenged 7 moves combo.
