Shaolin Xi Qi Wu Shu Guan, Zhong Guo, Henan, Dengfeng shi, Shaolin, Xi Guo Dian Cun.

Shaolin Xi Qi House of Martial Arts, China, Henan, DengFeng City, Shaolin, West Guodian Village.

That should be enough for Postal, Dengfeng is in Zhengzhou prefecture, but I don't know that its necessary to write that or not; 郑州专区
